Below are links to forms and information necessary to apply for Graduate school. Download the forms and complete them as instructed. The forms are in either Adobe Acrobat 
or Microsoft Word 
format. You will need access to the appropriate program to view them. If you have trouble downloading the application forms please complete the info request form, and we will mail the forms to you.
Pertinent Application Procedures
- A completed University Application Form is required by the Admissions deadline (shown below).
- A completed Supplemental Application Form and three letters of reference
are required by the Department.
- Original and certified documents must be submitted.
- TOEFL (min. 550) and GRE scores are required of all international
applicants.
- A $50.00 University processing fee is required.
- ALL information must be sent to the Department
at the address shown below. We will forward University forms to the
appropriate office.
- All complete applications are automatically considered by the Department
for assistantship and fellowships.
- Deadline for applications to be considered for financial aid:
- March 1st for Fall admission (classes start approx. 3rd week of August)
- October 1st for Spring admission (classes start 2nd week of January)
Incomplete Applications cannot be considered!
Please make sure your application materials and reference letters are sent promptly.
A Complete Application Package Must Include the Following (Originals)
- University application (by mail or you can access applications online on the WVU website )
- Supplemental Department Application Form (available for download)
- Three signed letters of reference (available for download)
- TOEFL scores
- GRE scores
- University processing fee
